Most ISM findings are documentation failures, not safety failures: a procedure nobody follows, a record nobody keeps, a drill logged but not run. A system written for your fleet size and trade avoids that.

Internal audit programme
Annual internal audit of vessel and office, findings graded by risk with corrective action, responsible person and target date, then verified at closure.
Revision control
Controlled document revisions, distribution to the fleet and evidence that the current version is the one in use onboard.
